Standing before its sleek console and robotic arms, I snapped a photo and posted it with the caption "I met da Vinci trainer. I made a new dream." Little did I know how that moment would shape my future.

Six years later, when I sutured a grape with a surgical robot for the first time, my hands were shaking. Not the robot's hands - those were steady as stone - but my own hands on the controls, guiding titanium fingers smaller than pencil tips as they pierced the delicate fruit skin. One wrong move, too much pressure, and the grape would burst. Just like, I imagined, something far more precious could one day burst under my robotic touch.

The path to becoming a pediatric robotic surgeon is paved with thousands of these tiny moments, each more challenging than the last. I still remember my first attempt at dissecting a practice gel - how it kept tearing under my uncertain touch, each hole a stark reminder that real surgery allows no room for error. But that’s why I chose this path. As someone fascinated by technology’s potential to transform medicine, I was drawn to how robotic systems can translate my movements into actions with unprecedented precision. Each repetition at the console, eyes fixed on 3D displays while my fingers learn this new language of precision. Each repetition brings me closer to my goal: performing a precise surgery that leaves barely a whisper on a patient’s body, rather than the bold signature of traditional open surgery.

Now, as I prepare for my first robotic pyeloplasty - a delicate procedure to repair obstructed kidney drainage in a young patient - each milestone feels like summit after summit on an endless mountain. But that’s exactly what drew me to robotic surgery. In a field where millimeters can mean the difference between healing and harm, there’s no such thing as “good enough” - there’s only the constant pursuit of perfection.

Looking ahead, I see a future where these precise, robot-assisted movements will make complex pediatric surgeries safer and less traumatic. Every grape sutured, every simulator mastered, brings that future one step closer.

How Artificial Intelligence is changing Pediatric Urology

Let’s talk about something that’s quietly transforming pediatric healthcare: artificial intelligence (AI). Think of AI like a highly intelligent assistant—it’s not here to replace doctors but to give them supercharged tools to make better decisions. Whether we like it or not, AI is making its way into healthcare.

So, What’s AI Doing in Pediatric Urology?

At its core, AI mimics aspects of human intelligence: learning from data, identifying patterns, and making predictions. In pediatric urology, this means improving diagnosis, refining surgical precision, and personalizing care. Let's break it down and get real about what's happening and why it's exciting.

Diagnosis: Spotting Problems Early

One of the most critical task in pediatric urology is to identify patients needing specialized care and interventions. Think of tools that can analyze imaging data without getting tired or distracted, and at the same time process massive amounts of data. These tools could act as a second opinion for doctors, helping to catch issues that might otherwise go unnoticed.

Recent studies showed that AI could significantly reduce diagnostic delays for hydronephrosis or vesicoureteral reflux (VUR). AI’s ability to highlight potential problems quickly is a game-changer in improving outcomes for children.

Surgery: Precision in Action

Robotic-assisted-surgery—using machines controlled by surgeons—is one of the most exciting advancements. Systems like the Da Vinci Surgical System enhance precision, using AI to optimize instrument movements and improve visualization. These systems allow surgeons to operate through small incisions while viewing magnified 3D images of the surgical site.

In the future it might be possible for AI to training by analyzing surgical videos to provide feedback and predict complications. Imagine an AI suggesting better angles or warning about potential bleeding. It’s not replacing surgeons but helping them perform at their best.

Postoperative Care: Personalizing Recovery

After the surgery is done, the journey isn't over. As the postoperative care is critical, an additional "helper" is most welcome. Think of a tool helping personalize the recovery plans by analyzing data from wearables, electronic health records, and patient feedback. It could flag potential complications early and recommend tailored follow-ups. And what about identifying families needing extra support to stick to recovery protocols, ensuring no child is left behind.

Current Applications of AI

And as we speak, AI tools are already making a difference. For example, Qure.ai analyzes radiological images to assist in identifying abnormalities, while the Da Vinci Surgical System uses AI to enhance surgical precision. Predictive analytics tools are tailoring postoperative care, although these applications are still in their early stages and used on very limited scale.

Challenges and Limitations

With all this excitement, AI isn’t without its hurdles. One major issue is the ‘black box’ problem, where AI systems make decisions without explaining how they arrived at them. For instance, an AI might flag a kidney anomaly but fail to clarify the reasoning, making it hard for doctors to trust or act on the recommendation. There are ethical concerns of using AI, risks of over-reliance, making sure that these systems make unbiased decisions.

Additionally, integrating AI into clinical workflows requires extensive validation and training. Pediatric urology, often dealing with rare conditions, lacks the large datasets AI needs for effective learning. AI also struggles with nuanced clinical judgments that require understanding individual patient histories and unique circumstances.

Why This Matters

For parents, AI means their child’s doctor could access cutting-edge tools and insights, regardless of location. For clinicians, AI offers support systems to enhance their already exceptional work. For healthcare innovators, it’s proof that AI is more than a buzzword—it’s a quiet revolution reshaping pediatric care.

The future of pediatric urology is bright, and AI is lighting the way.

The dance of decision

Amid the orchestrated chaos of a bustling hospital shift, moments stand out, painting stories of hope, despair, challenge, and triumph. On one such busy day, a case demanded my undivided attention – a few months old infant, the firstborn in their family, grappling with recurrent intussusception.

Intussusception, while treatable, becomes a challenge when it recurs. Conservative treatments had been attempted, but the child's suffering persisted. Faced with this predicament, I reached the heart-wrenching decision that surgery was the only viable course of action to alleviate the baby's pain.

The decision to operate, especially on someone so young, is never made lightly. The weight of potential complications, the mental rehearsals of every possible scenario – both good and bad – is a burden every surgeon knows all too well. As much as we strive for perfection, we are consistently reminded of our humanity and the unpredictability of the operating room. Over the years, I've come to understand that every surgeon, through trials and tribulations, garners invaluable lessons that shape their approach.

Before making that first incision, the entire procedure plays out in a surgeon's mind. Where should the cut be? What steps to follow? What pitfalls to avoid? These mental rehearsals are crucial, guiding us through the procedure while minimizing potential complications.

Yet, surgery is not a solitary endeavor; it's a symphony of collaboration. A surgical team's strength is not determined by its most skilled member but by its least experienced one. This understanding underscores the importance of shared knowledge. A great surgeon knows that hoarding knowledge is a disservice to the team and, ultimately, to the patient.

Fortune favored us that day. The surgery was successful, and the child's recovery was swift and smooth. Before long, the baby was ready to embrace the comfort of home again. A few days later, a heartfelt letter from the child's mother graced my desk. Words of gratitude, expressing the relief of a mother whose child was returned to health. It was a poignant reminder of why we brave the challenges of this profession.

Every patient, every surgery, every success, and every setback weave the intricate tapestry of a surgeon's journey. And amidst the highs and lows, it's moments like these – of gratitude and hope – that fuel our relentless pursuit of excellence.

The Lifelong Pursuit of Surgical Mastery

In the intricate dance of surgery, every movement, every decision, every incision matters. But the rhythm and flow of this dance don't remain constant; they evolve, shaped by experience, knowledge, and the ever-advancing tools of the trade.

I recall a recent conference I attended, nestled among peers and pioneers in the surgical field. As I absorbed the wealth of knowledge around me, a particular technique caught my attention. It was a slight modification to a procedure I'd performed countless times. Yet, this subtle change promised a better cosmetic outcome without extending the surgery's duration or complexity. Armed with this newfound knowledge, I returned to my practice, eager to apply what I'd learned. The results were immediately evident. Patients benefitted from an enhanced aesthetic outcome, a testament to the power of continuous learning.

Another course ushered in a more profound transformation. A seasoned expert introduced a novel approach to a familiar procedure. Adopting this method, I found that my patients experienced a swifter recovery, accompanied by significantly reduced pain. The joy of witnessing their comfort, seeing them bounce back faster and stronger, was unparalleled.

Such experiences underscore a fundamental truth: surgery is not a static craft. It is an ever-evolving art form, shaped by the collective wisdom and innovations of countless dedicated professionals. The tools, techniques, and methodologies of yesterday pave the way for the breakthroughs of tomorrow.

As the renowned author and surgeon Atul Gawande once said:

We always hope for the easy fix: the one simple change that will erase a problem in a stroke. But few things in life work this way. Instead, success requires making a hundred small steps go right - one after the other, no slipups, no goofs, everyone pitching in.

In the world of surgery, we must remember that our journey doesn't end when we leave the operating room. It continues with every course we take, every conference we attend, and every patient we treat. For in the pursuit of perfection, there is always another step to take, another lesson to learn.

The unique world of Pediatric Urology

Pediatric urology is a niche that beautifully marries two essential medical fields: pediatric surgery and urology. This subspecialty focuses on diagnosing and treating children's urinary and genital problems, and it demands not just a wide breadth of knowledge but also an unwavering commitment to excellence and keen attention to detail.

The Spectrum of Pediatric Urology

At its core, pediatric urology is about ensuring the best possible care for a child's urinary and reproductive health. The cases encountered can range from simple ones, such as hydrocele testis or phimosis, to more intricate and complex issues often seen in patients with conditions like spina bifida or bladder exstrophy.

The surgeries in this domain can be as technologically advanced as laparoscopic and robotic procedures or as delicate and detailed as the plastic surgeries required for conditions like hypospadias. Each procedure, regardless of its complexity, demands precision, skill, and a gentle touch. After all, the patients are children, and their bodies are still growing and developing.

Journey to Becoming a Pediatric Urologist

In Poland, the journey to becoming a pediatric urologist is a testament to the dedication and passion required in this field. Aspiring pediatric urologists first need to specialize either in pediatric surgery or general urology. Upon completing this rigorous training, they then embark on a minimum of a two-year fellowship dedicated solely to pediatric urology.

These additional years of specialized training are crucial. They provide budding pediatric urologists with the skills and knowledge they need to address the unique challenges that children present, be it anatomical differences, growth considerations, or the psychological aspects of treating younger patients.

The Craft Behind Pediatric Urology

Pediatric urology is a realm that demands a deep understanding of the intricacies associated with the urinary and reproductive systems of children. Unlike its counterpart, pediatric surgery, which encompasses a broader range of surgical interventions, pediatric urology hones in specifically on conditions related to the kidneys, bladder, ureters, and genitalia. This specialization calls for a delicate balance of technical expertise and judicious decision-making.

A pediatric urologist is armed with a diverse array of diagnostic tools at their disposal, from advanced imaging techniques to intricate urodynamic tests. With these tools, they can gain a comprehensive understanding of a child's condition. However, it's not just about diagnosis; the real challenge often lies in determining the best course of treatment for each unique case. With a myriad of possible interventions, from non-invasive therapies to detailed surgeries, making the right choice requires a blend of experience, knowledge, and intuition.

In Conclusion

Pediatric urology stands as a testament to the precision and depth required in medical subspecialties. Beyond the surgeries and treatments, it's about making decisions that will best serve each child's long-term health and well-being. For those who venture into this field, they don't just become surgeons; they become custodians of a future generation's health and happiness.

The Night Shift Diaries: Behind the scenes with a doctor

Ever wondered what a doctor's night shift really entails? Allow me to pull back the curtain on the nocturnal world of medicine.

Over the last six years, I've danced with the night - whether in a bustling hospital, a silent night clinic, or the unpredictable realm of ambulance services. Despite the varied settings, the essence remains consistent. I've long wanted to shed light on how these extended and often erratic hours weigh on a doctor's well-being.

Picture this: After a day filled with surgeries, patient check-ins, consultations, and endless paperwork, you'd think it's time to rest. But in the on-call room, luxury is a rare find. Sometimes you're treated to basic bedding; other times, it's just you and your trusty sleeping bag. You collapse, silently pleading for sleep, but rest is elusive.
"Sleep, sleep, sleep, please come..."- You wish falling asleep would be as easy as that. The weight of your responsibilities keeps your mind churning.
The night is punctuated with calls - emergencies, deteriorating patients, or consultations. Each summons finds you rising, tending, and hoping the next call is hours away. Sometimes, the operating theatre beckons, and you're acutely aware that the stakes are life-altering.

Dawn might be breaking outside, but your shift isn't over. There are patients to check on, reports to finalize, and while morning's adrenaline keeps you going, the fatigue eventually seeps in, often hitting hardest on the journey home.

Yet, not all nights are grueling. Some shifts end with the gratification of a life saved or a medical mystery unraveled. These moments fuel your spirit, reminding you why you chose this path.

The unpredictability of a 24-hour shift is its hallmark. Will it be calm or chaotic? Will sleep be a fleeting moment or a luxury? Or will there be that one case that lingers, questioning your every decision?

While night shifts are an inextricable part of healthcare, research underscores their health implications. From immediate cognitive challenges to long-term risks like depression, diabetes, and even certain cancers, the toll is undeniable. Poor post-shift habits can exacerbate these effects.

For years, I'd return home post-shift, aimlessly wandering my apartment, a shadow of my usual self. It took time to craft a restorative post-shift routine: a hearty breakfast, an uplifting show, meditation, and most importantly, undisturbed sleep. I've also found solace in the unwavering companionship of pets, echoing sentiments of a pediatric surgery professor who once shared the therapeutic presence of a furry confidant.

No magic formula exists. Some cases will always challenge our decisions, making us second-guess. But in those moments, we must remember: we gave our best in that moment. Our mission? To continually learn, evolve, and always strive for excellence.

Helmets: A simple choice that can save lifetimes

This post sprang from heartfelt conversations with my parents, both of whom are on the brink of their 60s. Their zest for life is evident in their daily fitness routines. My mom, an avid runner, clocks in about 7km during her hour-long morning runs. My dad, on the flip side, is a cycling enthusiast who also dabbles in general fitness.

During a recent visit, I noticed a crucial safety accessory missing: helmets. My dad initially brushed off my concerns, citing his careful driving and the perceived discomfort of helmets.

Here's the crux: Why is wearing a helmet so vital?

Helmets can reduce the risk of traumatic head injuries by a staggering 60%. When we're talking about the brain—an irreplaceable organ—the argument for helmets becomes compelling.

Poland registers one of the highest rates of cyclist fatalities in Europe, with approximately 300 lives lost annually due to bicycle-related accidents. It begs the question: How many of these tragedies could have been averted with the simple act of wearing a helmet? From my professional experience, I've encountered numerous heart-wrenching stories of bike accidents. Phrases like "he usually wore his helmet" or "she was just biking nearby" serve as somber reminders. While cycling, several external factors are beyond our control. Yet, choosing to wear a helmet is a decision entirely in our hands.

If more people embraced helmets, it might soon become an unspoken rule—a rule that has the potential to save countless lives.

After sharing these insights, I managed to persuade my dad to invest in a helmet. We visited a bike shop, allowing him to pick one that fit him best. And on our subsequent bike outing, my heart swelled with relief. Not only did he look dapper, but his head was also safely shielded.

Why #MedBikini is conquering the Internet.

Recently, an article titled "Prevalence of Unprofessional Social Media Content Among Young Vascular Surgeons" appeared in the Journal of Vascular Surgery.

The study involved authors sifting through publicly accessible social media accounts of vascular surgeons, pinpointing what they deemed as "unprofessional" content. Intriguingly, the data was gathered by three young male researchers, aged between 28 and 37, who set up fictitious social media accounts to monitor their subjects. The study's methodology did not specify clear inclusion or exclusion criteria. Notably, the authors categorized comments on topics such as abortion, gun control, and other pertinent political issues as "unprofessional."

However, it's essential to remember: doctors are human beings too. We experience emotions, engage in everyday activities, and, yes, even have our moments of leisure and relaxation, be it on a beach in a bikini or elsewhere. Does indulging in these ordinary facets of life undermine our professionalism within the confines of our work?

Double standards, especially those rooted in gender biases, have no place in our society. No individual, regardless of their profession or gender, should dictate what another should wear or how they should feel about it.

Being in the medical profession does not strip us of our rights as individuals. We have not just the right but also the responsibility to voice our opinions on pressing societal issues, be it social injustice, racial biases, discrimination, or laws that jeopardize safety. Speaking up on these matters doesn't dilute our professionalism; instead, it reinforces our commitment to holistic well-being and equity.

Shaping the Future: My year teaching anatomy

"The mediocre teacher tells. The good teacher explains. The superior teacher demonstrates. The great teacher inspires.”
― William Arthur Ward

Last year presented me with a cherished opportunity: teaching anatomy to first-year medical students. As I embarked on this journey, I was determined to deliver the best learning experience for them. Quickly, I realized that teaching doesn't just demand vast knowledge but also hinges on profound human connection skills.

Anatomy is the bedrock of medical education. It lays the foundational framework for every budding doctor and is indispensable. While today's digital age offers a plethora of books, apps, and resources, nothing can replicate the invaluable experience of direct engagement with specimens and guidance from seasoned experts.

The pandemic threw its fair share of challenges. Yet, I swelled with pride seeing my students remain steadfast in their dedication. Their perseverance amid adversity was commendable.

In my teaching approach, I prioritize support, encouragement, and fostering an environment where students can experiment without the looming shadow of undue stress. I reflected on myself and my experiences from the past. I also garnered numerous insights from the Anatomy Education Podcast.

My students, the future torchbearers of the medical profession, committed themselves wholeheartedly. It was deeply gratifying to witness the majority sail through their exams with flying colors. The anticipation of them donning the doctor's coat someday fills me with hope and excitement. Their aspirations, intertwined with the invisible threads of mentorship, serve as a testament to the profound bond that can form between a teacher and student. Watching them flourish is one of the most fulfilling experiences I've had.

Navigating the New Normal.

Your experience in these trying times is often shaped by where you work and your age. I'd like to offer a glimpse into the mind of a young female doctor working in a non-COVID department.

The heaviest burden we bear is the uncertainty. We grapple with the absence of established procedures, definitive guidelines, and most critically, adequate equipment. Gone are the days when gloves and masks were used freely. Now, each item is meticulously accounted for and monitored. Every individual who walks into our emergency department is potentially a carrier, many oblivious to their own status. We're in a daily battle with an unseen adversary that lurks in the air and on surfaces.

In the pre-pandemic era, my routine was markedly different. A single pair of gloves sufficed for each patient, and masks were the exception, not the rule. My attire was bright and cheerful scrubs adorned with characters like Mickey Mouse or fluttering butterflies. Disney pins embellished my uniform, and I always had a stash of stickers and toys to gift.

Today, I stand concealed behind a nondescript mask, a donation to our cause, and a face shield that often fogs up my glasses and muffles my voice. The whimsical scrubs of yore have been replaced by disposable ones, typically reserved for the operating theatre. Gone are the animated characters and vibrant stickers that once brought a glint to a child's eye. My current attire is dictated by pragmatism: everything I wear must either be discarded post-use or sterilized. The guiding principle is minimalism, retaining only the essentials.

Despite the whirlwind of changes, one constant remains: my unwavering commitment to being as compassionate and affable as possible. In today's context, I liken myself to an aspiring astronaut.

The reality is that this pandemic won't vanish overnight. It compels us to reconsider every facet of our lives, from daily routines to work protocols. We must innovate and adapt to minimize contamination risks, all while carving out our 'new normal'.

The Journey to Surgical Mastery

Imagine a world where certain books haven't made their way to Goodreads yet. These books might not be bestsellers or written in English, but they're treasures waiting to be discovered. "How I Became a Surgeon" (in Polish, "Jak stawałem się chirurgiem") is one such gem.

Curated by Prof. Bielecki, this book encapsulates a series of lectures delivered during a regional surgeons' meet in South-East Poland. It delves into the personal journeys of various professors and physicians as they navigated their paths to become surgeons. Their tales are replete with challenges, from the harrowing experiences of World War II to the repressions of communism, and personal battles. Yet, a common theme resonates throughout: the importance of perseverance, clarity of purpose, and unwavering focus on one's goals. After all, the road to greatness is seldom easy.

Prof. Bielecki emphasizes that a surgeon isn't merely a skilled craftsman. A surgeon also embodies the qualities of an internal medicine doctor, deftly blending the art and science of surgery. The attributes of a great surgeon include diligence, problem-solving prowess, a calm demeanor, and humility. Overconfidence has no place in the operating room. He believes that teaching is one of the best forms of learning, and the earlier one embarks on this journey, the better.

Communication is pivotal. Engaging with patients, visiting them regularly, observing, and being open to seeking assistance when faced with uncertainties are all integral to a surgeon's role. The value of a supportive team cannot be overstated – it not only makes the work more manageable and effective but also enhances the likelihood of positive outcomes.

One recurring sentiment among the speakers was the indispensable role of mentorship. The bond between a mentor and a student is paramount in shaping the latter's career. Observing veteran surgeons and exploring other clinics provide insights that are second to none. And the most crucial advice? Never cease the quest for knowledge.

The book poignantly concludes with a touching sentiment:

"When you operate on an elderly man, envision your father. And when you attend to a young patient, see in them your own child."

Life Behind the Scalpel

In today's digital age, they often say that if you're not on social media, you're nowhere. Well, consider this my grand entrance.

I am not just any surgeon; I specialize in pediatric surgery. My patients range from newborns to young adults, and I am deeply passionate about my work.

The joy I derive from my profession is not merely from the surgical procedures themselves, though I do find them enthralling. It's the smiles on the children's faces, the sighs of relief from their parents, and the sheer satisfaction of witnessing their recovery and helping them reach their milestones.

Being in the operating theater is an experience like no other. There's a unique ambiance – the sterile scent, the bright lights, the precision instruments laid out in preparation. It's not always pristine; surgeries can be intense, sometimes even chaotic.

Surgery, to me, is more than just a job. It's a delicate blend of art and science. Each procedure has its own rhythm, its own challenges. Sometimes it feels like crafting a masterpiece, and at other times, it's a fierce battle for a life, an organ, or even one's own composure. The suspense lingers until the very end, as the outcome remains uncertain until the last stitch is in place.
